Placements24 in Benoni is seeking an experienced Case Manager to join our social services team. This hybrid role combines remote work with in-person client visits to assess needs, develop service plans, and coordinate access to housing, healthcare, and counseling.

The ideal candidate has a Bachelor's degree in a related field, at least 3 years of case management experience, and strong interpersonal skills to advocate for clients across diverse communities in Gauteng.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or related field.

Responsabilités

• Conduct thorough client assessments to identify needs, strengths, and barriers to self-sufficiency.
• Develop individualized case plans in collaboration with clients, setting realistic goals and timelines.
• Coordinate and facilitate access to housing, employment, healthcare, and counseling services.
• Monitor client progress through virtual check-ins and in-person meetings.
• Maintain accurate case files, documenting interactions, progress and outcomes.
• Advocate for clients to ensure access to appropriate services across agencies.
• Liaise with external providers and government agencies to build a supportive network.
