Job summary

Placements24 in Polokwane is seeking a results-oriented Case Manager to join our community services team. This hybrid role involves managing client caseloads, assessing needs, and coordinating services to ensure positive outcomes for individuals and families.

You will work both remotely and in the field, providing direct support and connecting clients with resources, with a focus on empowering clients toward self-sufficiency within Limpopo.

Qualifications

• Degree in Social Work, Psychology, Sociology, or a related field.
• Proven experience as a Case Manager or in a similar social service role.
• Strong knowledge of social welfare systems and community resources in the Polokwane area.
• Excellent assessment, planning, and intervention skills.
• Proficiency in case management software and virtual communication tools.
• Ability to manage a hybrid work schedule effectively.

Responsibilities

• Assess client needs, develop individualized case plans, and monitor progress towards goals.
• Coordinate services with various agencies, including healthcare providers, educational institutions, and government support programs.
• Provide direct support and counseling to clients, addressing social, emotional, and practical challenges.
• Maintain accurate and comprehensive client records, ensuring confidentiality and compliance.
• Facilitate client access to community resources and advocate on their behalf.
• Conduct regular case reviews and participate in team meetings, both virtual and in-person.
