You will assess needs, develop intervention plans, provide counseling, advocate for clients, and coordinate with local resources to promote well-being and social integration.

Qualifications

• A recognized degree in Social Work and SACSSP registration.
• Minimum of 2 years' post-qualification experience in community-based social work.
• Demonstrated experience in case management, counseling, and advocacy.
• Proficiency in using case management software and standard office applications.
• Excellent interpersonal, communication, and problem-solving skills.
• Ability to work independently and manage a caseload effectively, with strong organizational skills for the hybrid work model.

Responsibilities

• Conduct comprehensive assessments of client needs, identifying social, emotional, and environmental challenges.
• Develop, implement, and monitor individualized case management plans.
• Provide counseling and therapeutic interventions to clients and their families.
• Advocate for clients' rights and connect them with community resources and services.
• Maintain detailed and confidential client records, documenting all interactions and progress.
