Job summary

Placements24 is seeking a compassionate Social Care Support Worker to join our team in Pretoria. This hybrid role combines fieldwork with remote administrative tasks to support individuals and families facing social challenges, helping them live more independently and improve their quality of life.

You will work with a team of professionals, advocating for clients' needs and connecting them to community resources while maintaining confidentiality and ethical practice.

Qualifications

• Relevant qualification in social work, community development, or healthcare support.
• Experience with vulnerable populations preferred.
• Understanding of ethical practice and confidentiality in social care.
• Strong interpersonal skills and the ability to build rapport with diverse clients.
• Driver's license and access to a reliable vehicle for fieldwork.
• Proficiency in basic computer skills for remote record-keeping and communication.

Responsibilities

• Provide direct support and assistance to clients in homes and communities, based on individualized care plans.
• Assist clients with daily living activities, such as personal care, meal preparation, and medication reminders.
• Facilitate client participation in community activities, appointments, and social events.
• Monitor client well-being and report any changes or concerns to the supervising social worker or case manager.
• Help clients develop and maintain life skills, fostering independence and self-reliance.
• Maintain accurate and confidential records of client interactions and progress, both in the field and remotely.
