Job summary

Placements24 is seeking a compassionate Social Support Officer in Centurion to provide direct support and case management to individuals and families facing social challenges. Qualifications

• A degree or diploma in Social Work, Community Development, or a related discipline.
• Demonstrable experience in case management or social support roles.
• In-depth knowledge of social welfare policies and community resources in Centurion and surrounding areas.
• Exceptional interpersonal, communication, and conflict resolution skills.
• Ability to handle sensitive information with discretion and maintain professional boundaries.

Responsibilities

• Conduct client assessments to identify needs and eligibility for social services.
• Develop and manage individualized support plans, tracking progress and outcomes.
• Facilitate referrals to housing, healthcare, employment, and other support services.
• Provide counseling and emotional support to clients and their families.
• Maintain detailed client records and prepare reports for management and funding agencies.
• Advocate on behalf of clients to ensure their rights and needs are met by service providers.
