Qualifications

• Diploma or Bachelor's degree in Social Work, Psychology, Occupational Therapy, Disability Studies, or related field.
• Proven experience working with individuals with disabilities and their families.
• Thorough understanding of disability legislation, services, and support systems.
• Excellent communication, listening, and empathetic skills.

Responsibilities

• Assess the needs of individuals with disabilities and develop personalized support plans.
• Connect individuals with appropriate community resources, services, and assistive technologies.
• Provide information and guidance on available benefits, rights, and entitlements.
• Coordinate with healthcare providers, therapists, educators, and other professionals to ensure integrated support.
• Advocate for the rights and needs of individuals with disabilities within various settings.
• Facilitate social inclusion activities and community participation.
• Provide emotional support and counseling to individuals and their families.
• Monitor the effectiveness of support plans and make adjustments as needed.
