Qualifications

• Relevant qualification in Social Work, Occupational Therapy, Psychology, or Disability Studies.
• Minimum 3 years' experience in disability support or case management.
• Strong knowledge of disability types, resources, and local Kimberley/Northern Cape.
• Excellent communication, interpersonal, and advocacy skills.
• Ability to develop and manage individualized support plans.
• Strong organizational and problem-solving abilities.
• Experience with diverse client groups and inclusion.
• Proficient in case management software and digital docs.
• Valid driver’s license and willingness to travel within Northern Cape.
• Commitment to person-centered approaches and empowerment.

Responsibilities

• Conduct comprehensive needs assessments for individuals with disabilities.
• Develop, implement, and review individualized support plans with clients and stakeholders.
• Coordinate access to healthcare, therapy, education, and employment services.
• Provide information, advocacy, and guidance to clients and families.
• Build relationships with community organizations and government agencies in Northern Cape.
• Monitor and adjust support plans as needed.
• Organize peer support groups and community integration activities.
• Maintain confidential client records and ensure privacy compliance.
• Respond to client needs and concerns promptly.
• Promote inclusion, respect, and independence for clients.
