Qualifications

Responsibilities

• Conduct comprehensive needs assessments for individuals with disabilities.
• Develop, implement, and review person-centered support plans.
• Coordinate and arrange services, including therapy, personal care, and community access.
• Liaise with families, caregivers, and other support network members.
• Advocate for the rights and needs of participants.
• Ensure compliance with relevant legislation and NDIS guidelines (if applicable).
• Manage budgets and service agreements effectively.
• Facilitate communication between participants and service providers.
• Maintain accurate and up-to-date client records.
• Respond to queries and resolve issues related to support provision.
