Qualifications

• A relevant qualification in disability studies, social work, or a related field.
• Demonstrated experience coordinating support services for individuals with disabilities.
• In-depth knowledge of disability legislation, support models, and available community resources.
• Strong interpersonal and communication skills, with the ability to build rapport with diverse individuals.
• Excellent organizational and problem-solving abilities, with meticulous attention to detail.
• Proficiency in relevant software for case management and reporting.

Responsibilities

• Conduct comprehensive needs assessments for individuals with disabilities to understand their specific support requirements.
• Develop individualized support plans in collaboration with clients, families, and relevant healthcare and support professionals.
• Coordinate the provision of various services, including personal care, therapy, assistive technology, and community participation.
• Liaise with service providers, government agencies, and community organizations to ensure seamless service delivery.
• Monitor the effectiveness of support plans and make adjustments as necessary to meet client needs.
