Job summary

Placements24 is seeking a dynamic Disability Support Program Facilitator in Tembisa, Gauteng. You will assess client needs, connect them to resources, and support service access to promote independence and inclusion.

In this on-site role, you will develop action plans, deliver workshops, build relationships with providers, and advocate for clients while maintaining confidential records. Travel within the area is required, and professional development opportunities are provided.

Qualifications

• A relevant degree in social work, community development, disability studies, or related field.
• 2+ years of experience in disability support, case management or community outreach.
• Knowledge of disability types, support services and SA legislation.

Responsibilities

• Assess support needs and develop personalized action plans.
• Facilitate access to community resources, healthcare, education and employment.
• Provide guidance and support to clients and families throughout their journey.
• Develop and deliver workshops on disability awareness and rights.
• Build relationships with service providers, government agencies and community groups.
• Advocate for clients to ensure rights and needs are met.
• Assist clients in developing life skills and self-advocacy.
• Monitor progress and adjust plans as needed.
• Maintain accurate confidential case notes and records.
• Collaborate with team to identify service gaps and propose improvements.
