Job summary

Placements24 is seeking a compassionate Disability Support Officer in Polokwane, Limpopo, to empower individuals with disabilities. You will assess needs, develop individualized support plans (ISPs), and advocate for client rights to enable independent living.

Collaborating with families, healthcare professionals, and community services, you will promote inclusion and social participation, ensuring access to resources and support networks.

Qualifications

• Relevant qualification in social work, community development, psychology, occupational therapy, or related field.
• 2–3 years of experience in disability support or related social care role.
• Solid understanding of disability types, support needs, and South Africa legislation.
• Excellent communication, active listening, and problem-solving skills.
• Ability to build rapport and empower clients.
• Ability to work independently and as part of a supportive team.
• Basic IT skills for record-keeping and communication.

Responsibilities

• Conduct needs assessments to understand client requirements and goals.
• Develop and review individualized support plans with clients and families.
• Provide direct support with daily living activities to promote independence.
• Facilitate client participation in community programs and opportunities.
• Advocate for clients' rights in decision-making processes.
• Coordinate services with healthcare professionals and community agencies.
• Monitor progress and adjust support plans as needed.
• Maintain confidential client records and documentation.
• Provide information about services to clients and families.
• Promote awareness of disability issues in the community.
