Job summary

Placements24 is seeking a compassionate Community Support Worker to join our team in Johannesburg, Gauteng. You will provide essential assistance to vulnerable adults, supporting daily living activities and fostering independence within a caring, professional environment.

You will develop personalised care plans, offer emotional support, and help clients participate in community activities while maintaining accurate records and collaborating with a multidisciplinary team.

Qualifications

• Proven experience working with vulnerable adults or in a similar community support role.

Responsibilities

• Provide direct support and assistance to vulnerable adults in daily living activities including personal care, meal preparation, and household tasks.
• Develop and implement personalised care plans in collaboration with individuals, families, and healthcare professionals.
• Offer emotional support, companionship, and encouragement to clients, promoting dignity and independence.
• Facilitate participation in community activities, social outings, and appointments; ensure clients stay connected and engaged.
• Monitor and report on client progress, changes in condition, and any concerns to the supervisor or healthcare team.
• Maintain accurate and confidential records of client interactions, care provided, and outcomes achieved.
• Adhere to organisational policies, procedures, and ethical guidelines to ensure safety and wellbeing of clients.
• Collaborate with a multidisciplinary team to implement holistic care delivery.
