Qualifications

• A relevant qualification in Social Work, Community Development, Psychology, or a related field is highly desirable.
• Proven experience in community work, social care, or a similar role.
• Excellent communication, interpersonal, and active listening skills.

Responsabilités

• Conduct comprehensive needs assessments for individuals and families seeking assistance.
• Develop and implement individualized support plans tailored to client needs.
• Provide emotional support, counseling, and practical guidance to clients.
• Facilitate access to essential services, including housing, healthcare, and employment opportunities.
• Organize and lead community outreach programs and workshops.
• Maintain accurate and confidential client records and documentation.
• Collaborate with other community organizations and stakeholders to enhance service delivery.
• Advocate on behalf of clients to ensure their rights and needs are met.
• Monitor client progress and adjust support plans as necessary.
• Respond to crisis situations and provide immediate support and referrals.
