Job summary

Responsibilities
- Conduct client assessments to determine needs and eligibility for services.
- Provide individual and group support sessions to address social and emotional needs.
- Assist clients in accessing housing, healthcare, and employment support.
- Develop and implement individualized support plans with clients.
- Advocate for clients' rights with agencies and service providers.
- Maintain confidential client records and case notes.
