Responsibilities

• Assess client needs and provide information on available community resources and services.

• Conduct home visits and follow-ups to ensure clients are receiving adequate support.
• Provide advocacy for clients facing complex challenges.
• Develop and maintain strong working relationships with local service providers and agencies.
• Organize and facilitate community workshops or information sessions on relevant topics.
