Seamless Assist is seeking a Cardiology Patient Navigator (Coordinator) to support a U.S.-based cardiology team. The role emphasizes patient communication, scheduling, care coordination, and meticulous clinical documentation across a remote work setting.

The ideal candidate has a clinical background (CNA, MA, RN or equivalent), 3+ years in healthcare or telehealth, and bilingual English/Spanish proficiency. You will manage a high-volume caseload and coordinate with providers within U.S.

Formación

• Clinical healthcare background required (CNA, MA, RN or equivalent credential).
• 3+ years of experience in healthcare, telehealth, or medical office settings.
• Experience in care coordination or care management, preferably in telephonic or remote settings.
• B2+ English proficiency and B2+ Spanish proficiency (bilingual required).
• Experience using EMR/EHR systems.

Responsabilidades

• Patient Communication & Scheduling: Manage patient scheduling, reminders, and follow-up workflows in EMR/EHR systems.
• Care Coordination: Track testing schedules, follow-up appointments, and remote patient monitoring workflows; coordinate across a caseload of 150+ patients.
• Documentation & Compliance: Maintain accurate clinical documentation and ensure HIPAA compliance; support billing documentation as needed.
