Seamless Assist is seeking a Cardiology Patient Navigator to support a busy U.S.-based cardiology team. This remote role emphasizes patient communication, scheduling, care coordination, and clinical documentation. A clinical background and telehealth experience help manage a high-volume caseload with warmth and precision.

The ideal candidate is bilingual (English/Spanish), comfortable with EMR/EHR systems, and available during U.S. CST hours to coordinate care across cardiology pathways.

Formación

• Clinical healthcare background including CNA/MA/RN or equivalent.
• 3+ years in healthcare, telehealth, or medical office settings.
• Bilingual English/Spanish (B2+).
• Experience using EMR/EHR systems.
• Strong patient-centered communication.
• Willingness to work U.S. CST hours.
• Able to manage 150+ remote patients.

Responsabilidades

• Manage patient scheduling, reminders, and follow-up workflows in EMR/EHR systems.
• Conduct telephonic or remote patient outreach to reinforce care instructions and coordinate next steps.
• Support patients navigating cardiology care pathways, including CHF management protocols.
• Track testing schedules and remote monitoring workflows.
• Coordinate with the clinical team to ensure continuity of care across 150+ patients.
• Escalate clinical concerns within scope (no diagnosing or prescribing).
• Maintain HIPAA-compliant documentation and assist billing documentation as needed.
