Seamlessassist is seeking a Cardiology Patient Navigator (Coordinator) to support a U.S.-based cardiology team. This remote role focuses on patient communication, scheduling, care coordination, and clinical documentation.

The ideal candidate has a clinical background (CNA/MA/RN or equivalent), 3+ years in healthcare or telehealth, and bilingual English/Spanish. US business hours (CT) are required.

Formación

• Clinical healthcare background: CNA, MA, RN, or equivalent credential.
• 3+ years of experience in healthcare, telehealth, or medical office settings.
• Experience in care coordination or care management, preferably in telephonic or remote settings.
• B2+ English proficiency | B2+ Spanish proficiency (bilingual required).
• Experience using EMR/EHR systems.
• Strong patient-centered communication skills.
• Ability to work U.S. business hours (CT).
• Strong written and verbal English communication; assessed at screening.

Responsabilidades

• Patient Communication & Scheduling: Manage patient scheduling, reminders, and follow-up workflows in EMR/EHR systems.
• Care Coordination: Track testing schedules, follow-ups, and remote patient monitoring workflows; coordinate with clinical team for 150+ patients; escalate concerns as needed.
• Documentation & Compliance: Maintain timely clinical documentation; ensure HIPAA compliance; support billing documentation.
