Job summary

WellLife Network is seeking a Youth Care Coordinator to establish supportive relationships with youth and families managing behavioral and health conditions. The role involves assessments, care planning, and field outreach to improve health and social functioning in Suffolk County, NY with possible Nassau assignments.

Responsibilities include coordinating care with multi-disciplinary teams, maintaining records, and ensuring access to services while adhering to DOH HHSC guidelines and training

Qualifications

• Must have strong communication and interpersonal skills.
• Must maintain CANS-NY certification.
• Must maintain valid driver’s license.

Responsibilities

• Establish supportive relationships with youth and families to improve health and social functioning.
• Conduct initial and ongoing assessments (CANS-NY, Complex Trauma, Comprehensive) to identify needs.
• Develop and revise care plans with member goals and wellness focus.
• Maintain monthly contact via phone or in-person outreach; provide field-based services.
• Coordinate with healthcare providers, insurers, and community resources.
