Job summary

WellLife Network in Coram, NY is seeking a Mental Health Worker to provide access, advocacy, and supports for youth aged 5-21 at risk of serious emotional disturbance. You will work in Suffolk County to help families live successfully in the community.

Responsibilities include assessments, care planning, coordinating with providers, case documentation, and discharge planning. A flexible schedule with two evenings weekly is required.

Qualifications

• Bachelor's degree with two years of relevant experience or Master's degree with one year.
• Experience in care coordination, mental health, youth and families or related populations.

Responsibilities

• Provide service access, advocacy and supports for ages 5-21 with or at risk of SED.
• Conduct initial and ongoing assessments and develop care plans.
• Maintain records, coordinate care, and work with a multidisciplinary team.
• Engage families in discharge planning and ensure access to resources.
• Maintain a flexible schedule, including two evenings per week.
