WellLife Network is seeking a Youth Care Coordinator to build supportive relationships with youth (ages 5–21) and families, coordinating care to improve health outcomes in Coram, NY. The role includes ongoing assessments, care plan development, and linkage to medical and community resources.

You will collaborate with a multidisciplinary team to ensure timely, goal-directed services, maintain detailed EMR records, and participate in regular supervision and trainings to meet DOH/Hud HHSC

Formación

• Bachelor’s or equivalent degree; RN license with experience; Master’s with experience.
• Must maintain CANS-NY certification and a valid driver’s license.
• Two years of relevant care coordination experience or related field.

Responsabilidades

• Establish supportive relationships with youth and families to improve health and social functioning.
• Conduct initial and ongoing comprehensive assessments to determine needs and strengths.
• Develop and revise care plans focusing on health maintenance and wellness.
• Maintain monthly contact with members via in-person or telephonic outreach.
• Coordinate care, insurance linkage, and access to community resources.
• Collaborate with multidisciplinary team to ensure goal-directed care planning.
• Document thoroughly in electronic medical records and ensure accurate billing.
