Job summary

WellLife Network in Coram, NY is seeking a Senior Youth Care Coordinator to lead the Youth Care Management program. You will supervise a team of coordinators, ensure accurate documentation, and help maintain program policies and operations.

The role reports to Program Coordinators, with responsibilities spanning policy implementation, staff development, and collaboration with health home services to support youth and families.

Qualifications

• Licensed MS-level human services professional.
• Experience with Health Home Serving Children program.
• Strong leadership and communication skills.
• Program development experience and supervisory abilities.

Responsibilities

• Supervise 3–5 Youth Care Coordinators.
• Ensure documentation in client charts is complete and timely.
• Maintain policies, procedures and program operations.
• Provide staff guidance and training on health home requirements.
• Attend intakes and supervise field visits as needed.
• Oversee revenue monitoring and program policy implementation.
