WellLife Network in New York seeks a dedicated Care Manager to support individuals and families across the Huntington Station region. You will conduct outreach, complete assessments, and coordinate services to ensure quality, holistic care.

Qualifications

• Bachelor’s degree in Human Services or related field.
• Two years’ experience in human services required.
• New York State driver’s license and access to a vehicle.
• Ability to conduct outreach and assessments.
• Strong communication and record-keeping skills.

Responsibilities

• Conduct outreach activities to engage individuals with chronic medical conditions, mental health issues, or substance use disorders.
• Perform initial and ongoing comprehensive assessments to determine needs and goals.
• Prepare and revise care plans focusing on health and wellness.
• Maintain contact with clients monthly via phone and in person.
• Coordinate enrollment in medical insurance and linkage to providers and community resources.
• Collaborate with multidisciplinary team to ensure goal-directed care planning.
• Provide crisis intervention when needed and follow up accordingly.
• Maintain detailed EMR records and timely documentation.
• Attend weekly team meetings and report on client status and needs.
• Coordinate daily schedules with supervisor and field staff.
• Share resources and consult on medical/psychosocial issues as needed.
