Job summary

Child & Family in Cranston, RI seeks a Youth Support Specialist to provide case management and support Independent Living Program clients in a trauma-informed environment. You will promote health and help clients develop life skills toward self-sufficiency in a hybrid role with some remote work.

Responsibilities include intake assessments, developing wraparound plans, documenting incidents, supporting employment/education, and coordinating with state agencies and partners.

Qualifications

• High school diploma or equivalent; preferred degree in social work/psychology.
• Minimum 1 year experience with children/adolescents.
• Ability to work weekends and on-call as needed.
• Travel statewide is required.
• Bilingual in Spanish preferred.
• Strong organizational, critical thinking, and time-management skills.
• Basic computer skills; Microsoft Office 365 knowledge.

Responsibilities

• Provide case management services and support Independent Living Program clients.
• Promote physical, mental, and behavioral health per support plan.
• Collaborate to provide wraparound services and life-skills development.
• Maintain client records and documentation in a timely manner.
• Coordinate intakes and participate in meetings with state personnel and partners.
• Transport residents using agency vehicle per policies.
