Job summary

Child & Family Development is seeking a Youth Support Specialist to provide case management and wraparound services in a trauma-informed environment, supporting Independent Living Program clients to promote life skills and eventual self-sufficiency. The role includes collaboration with the agency and community partners to ensure comprehensive care.

The position is a Hybrid role with 20% remote work, requiring weekend and on-call availability, and state-wide travel as needed.

Qualifications

• Must have strong organizational, critical thinking and time-management abilities.
• Basic computer skills including Microsoft Office 365 are required.
• Ability to communicate effectively with diverse populations and collaborate with team members.

Responsibilities

• Provide case management services and support the Independent Living Program clients.
• Promote physical, mental, and behavioral health per the support plan.
• Develop wraparound services helping clients gain life skills for self-sufficiency.
• Maintain client records and timely documentation.
• Coordinate intakes with state agencies and participate in support meetings.
