Job summary

Family Service of Rhode Island seeks a Community Services Case Manager for Hope Street in Providence to support treatment goals for youth aged 4–19 and their families through intensive home and community-based services.

You will advocate for access to resources, collaborate with DCYF, courts, schools, and other providers, and work with a clinical team to improve clients’ lives. Bilingual abilities are valued and some evenings may be required.

Qualifications

• Bachelor’s degree in a human services field required.
• Experience operating medical record software, word processing, and databases.
• Spanish speaking preferred; bilingual/multilingual skills compensated.
• Flexible schedule with at least two evenings a week.
• Excellent multitasking and communication skills.
• Ability to work independently and as part of a team.

Responsibilities

• Support treatment goals for youth (ages 4–19) and families in home/community-based services.
• Advocate for clients to access resources from Human Services and similar agencies.
• Collaborate with referral sources (DCYF, Court, FCCP) and other providers.
• Advocate for clients to access appropriate resources to improve quality of life.
