Job summary

Community Solutions Inc. in Rhode Island seeks a Residential Re-entry Specialist/ Youth Residential Mentor to support youth residents, develop individualized plans, and guide reintegration into communities. You will document progress, ensure resources, and maintain calm, compassionate oversight throughout shifts.

The role emphasizes trauma-informed care, collaboration with the Program Director, and ongoing training to support client success in a multidisciplinary setting.

Qualifications

• High School Diploma (or equivalent) and 4 years experience in social services or criminal justice.
• Bachelor's degree plus one year experience preferred; substitution with relevant experience allowed.
• Experience with Youth Population preferred.

Responsibilities

• Provide direct care and supervision to youth residents in a residential setting
• Develop and implement individualized plans for each resident, in collaboration with other team members
• Provide ongoing support and guidance to residents throughout their stay
• Maintain accurate records of resident progress and behavior
• Ensure that all residents receive the necessary support and resources to reintegrate into their communities
• Conduct employment, job search and home confinement spot checks on each shift
• Conduct common area, room searches and perimeter searches as assigned and document results
• Conduct pat/pocket searches on all clients entering the facility and document results
• Complete house counts every hour from Midnight to 8am and every 2 hours from 8am to 10pm and document results
• Transport Clients as needed in a Company vehicle
• Conduct monthly emergency drills
• Secure urines from same sex clients and complete all required paperwork and electronic documentation
• Supervise all client visitation. Sign all visitors and contractors in and out of the facility
• Complete intake on all new arrivals prior to the end of your shift. Facilitate client orientation as assigned
• Facilitate all client movement in and out of the facility
• Monitor clients self-administration of medication
