Oasis Center in Nashville seeks an Outreach Case Manager to join the Street Outreach program. You will perform street canvasing, assessments and safety planning for homeless/street youth in the Metro Nashville area, coordinating entry to housing and needed services.

The role emphasizes trauma-informed care, anti-oppression practice and youth development, with responsibilities spanning referrals, documentation and community collaboration.

Qualifications

• Ability to effectively communicate in oral and written form.
• Knowledge of trauma-informed care and anti-oppression practices.
• Ability to navigate community resources and referrals.
• Experience with youth services and case management.

Responsabilités

• Provide screening, assessments and safety planning for homeless youth.
• Conduct street outreach canvasing and case management for RHSY.
• Assist youth in housing planning and documentation.
• Facilitate referrals and coordinate with partners; document interactions.
• Participate in staff meetings and supervision; maintain ethical records.
• Support Drop-In Center operations and community donations.
