Qualifications

• 2+ years' experience working with homeless individuals with mental illness.
• Experience with street outreach and case management is preferred.

Responsibilities

• Case Management of a defined caseload under supervision.
• Provide street outreach to engage unsheltered individuals.
• Conduct intake and mental health assessments and develop service plans.
• Engage landlords and assist with housing placement and Rapid Rehousing processes.
• Document services, complete HMIS entries, and maintain records.
