The Encore is seeking a dedicated Case Manager to support youth facing housing instability and related challenges. You will conduct comprehensive assessments, craft individualized service plans, and coordinate services across housing, health, education, and employment supports.

Ideal candidates bring strong communication, organizational skills, and a trauma-informed, youth-centered approach to deliver high-quality services in a fast-paced environment.

Qualifications

• Min. two years of case management with youth or vulnerable populations.
• Experience with intake, assessment, service planning, and progress monitoring.
• Ability to maintain confidential records and operate HMIS and funder requirements.

Responsabilités

• Conduct intake, eligibility, risk and needs assessments; develop individualized service plans with goals and timelines.
• Review participant progress; update plans and address barriers to achieve outcomes.
• Maintain confidential case notes, assessments, referrals, and discharge documentation.
• Enter participant data into HMIS and internal systems by established deadlines.
• Monitor eligibility, service utilization, and outcome measures to meet program targets.
• Identify complex cases and coordinate with supervisor; document actions and follow-up plans.
• Advocate for participants with providers, schools and community partners.
• Provide direct services and ensure high-quality, ethical service delivery.
