Job summary

Covenant House New York is seeking a dedicated Case Manager to deliver direct, supportive services to sexually and physically vulnerable youth in a short‑term emergency shelter. You will work with a multidisciplinary team to promote housing stability and personal growth for 16‑24 year olds.

Reporting to the Program Coordinator, you will develop individualized service plans, document progress, and advocate for youth with community providers while maintaining compliance with regulatory standards.

Qualifications

• Counsel young adults and provide case management services.
• Knowledge of trauma-informed care and resiliency-building.
• Ability to manage multiple projects and hearings with youth.
• Maintain confidential records and documentation in ISPs and service plans.
• Flexibility to work evenings, weekends, and holidays when needed.

Responsibilities

• Provide integrated counseling and case management to youth.
• Collaborate to develop individual service plans and track ISP progress.
• Meet with youth regularly to monitor goals and progress.
• Advocate for youth with external providers and services.
• Maintain confidential records and ensure regulatory compliance.
• De‑escalate crises and respond to emergencies per agency protocol.
