Job summary

Covenant House New York is seeking a compassionate Case Manager to provide direct services in our short-term Youth Shelter, working within an integrated, multidisciplinary team.

You will develop individual service plans, offer counseling and support, de-escalate crises, document progress, and advocate with external providers to promote housing stability and well-being for young people.

This role requires flexibility, weekend/evening hours as needed, and a strong commitment to trauma-informed care.

Qualifications

• Bachelor's degree preferred; otherwise HS diploma/GED with 1 year residential experience preferred.
• Experience with resiliency, strength-based youth development, and trauma-informed care is required.
• Strong multi-tasking and problem-solving abilities; upbeat demeanor.
• Experience working with diverse groups is required.

Responsibilities

• Provides integrated counseling and case management services engaging young adults to promote growth, housing stability, and service plan goals.
• Collaborates with youth to assess strengths and challenges and develop an ISP.
• Meets with youth regularly to review progress on ISP goals.
• Advocates on behalf of youth with outside service providers and collateral services.
• Creates and maintains confidential records and logs for youth, documenting interventions and collateral information.
• Ensures youths’ files comply with all regulatory requirements.
• Completes incident reports per agency policies.
• De-escalates crises and attends to emergencies per agency protocol.
