Job summary

Qualifications

• Minimum of an Associate degree with relevant work experience.
• Bachelor's degree in education, human services, or related field preferred; demonstrated case management skills and data management experience.

Responsibilities

• Monitor DL&T service contracts and ensure compliance with benchmarks.
• Maintain and update client files accurately; collaborate with multidisciplinary team.
• Recruit and retain program participants; facilitate wraparound meetings and resource sharing.
• Provide direct services in a drop-in center, including intake, assessment, and follow-up.
• Deliver trauma-informed crisis intervention and de-escalation support for youth and families.
