Job summary

Community Care Alliance is seeking an ACT Case Manager in Woonsocket, RI. This full-time role supports individuals with serious mental illness and co-occurring substance use disorders in their homes, promoting recovery and reducing crises.

You will assess clients, provide crisis intervention, and work within a multidisciplinary team to design treatment plans and connect them with community resources. A BA/BSW is required and transportation is necessary.

Qualifications

• BA/BSW in a human services related field is required.
• Knowledge of substance abuse and mental health is a plus.
• Experience with Electronic Health Record systems is preferred.
• Approved driver’s record and reliable transportation required.

Responsibilities

• Provide comprehensive community support to individuals living in their homes.
• Assess and evaluate clients on an ongoing basis and provide crisis intervention as needed.
• Develop, design, and implement treatment goals/plans to promote recovery.
• Collaborate with community resources to promote recovery and better mental health.
