Job summary

Community Care Alliance in Woonsocket, RI, is seeking a Case Manager Housing Specialist (Full Time) to support adults with mental health and addiction challenges in the community. You will assess housing needs, assist clients in securing safe, affordable homes, and remove barriers to care.

Responsibilities include connecting clients to emergency shelter, basic needs, benefits navigation, and coordinating primary care to improve functioning and stability.

Qualifications

• Associate or bachelor’s degree in psychology, social work, or a related human services field, with at least one year of relevant experience.
• Experience with individuals living with mental illness, substance use, chronic health issues, and/or homelessness.

Responsibilities

• Provide services to adults with mental health and addiction challenges to maintain and improve their level of functioning in the community.
• Conducts comprehensive housing assessments and assists clients in finding and maintaining safe, affordable housing.
• Connect to emergency shelter and housing, access basic needs, coordination of primary care, navigation of benefits/entitlements and legal issues, and building social supports.

Education

Associate or bachelor's degree in psychology, social work, or a related human services field, with at least one year of relevant experience.
