Job summary

Mission Prep Healthcare, part of the AMFM Healthcare family, is seeking dedicated Community Coordinators to support adolescents in residential treatment facilities. You’ll join a mission-driven team, providing compassionate care, safety, and encouragement within a supportive environment.

The role emphasizes collaboration with clinical staff, adherence to wellness concepts, and ongoing development within a culture that values trust, connection, and patient-centered care.

Qualifications

• Education: High School Diploma or GED, Bachelor's preferred.
• Background Check: Must pass ADP Selective Screening Services and other Federal/State background checks.
• Valid Driver’s License: 3+ years driving, 21+, clean record per policy.
• Physical Requirements: Computer work, walking/standing, lift up to 20 lbs., stairs navigation.

Responsibilities

• Coordinate and transport clients to daily activities, ensuring timely arrival and safety.
• Use interventions and communication skills with clients and families daily and in crisis.
• Adhere to Joint Commission policies and maintain environment safety and cleanliness.
• Assist with medication management coordination and documentation with clinical team.
