Responsibilities

• Coordinates and transports clients to daily activities, ensuring on-time arrival and safety.
• Supports medication management with clinical staff and documents meds.
• Maintains clean, hygienic residences and surroundings.
• Engages with clients to foster a connected and harmonious community.
• Documents activities in BestNotes/EHR and supports care plans.
