Job summary

Mission Prep Healthcare, part of the AMFM Healthcare family, is hiring a Community Coordinator to support adolescents in residential treatment facilities. The role involves coordinating transport, supervising activities, and assisting the clinical team with medication management.

The position emphasizes compassionate care and teamwork to foster healing and safety. You will work in a collaborative setting with management and clinical staff, following Joint Commission guidelines and MA health

Qualifications

• Education: High School Diploma or GED, Bachelor's preferred.
• Background Check: Must pass ADP Selective Screening Services and other Federal/State background checks.
• Valid Driver’s License: 3 years driving, 21+, insured by company policy, up to 3 moving violations in 3 years, max 1 major violation.
• Physical Requirements: Lift up to 20 lbs., walk/stand, navigate stairs.

Responsibilities

• Coordinates and transports clients to daily activities, ensuring on-time arrival and safety.
• Uses intervention and communication skills with clients and families daily and in crisis.
• Adheres to Joint Commission policies and infection prevention procedures.
• Collaborates with nurse and clinical team on medication management and documents medications per policy.
• Maintains residence cleanliness and a safe, respectful environment for clients.
• Acts as part of the Mission Prep community, supporting wellness journeys for youths.
• Documents activities in the electronic health record and uses BestNotes system.
• Conducts routine observation rounds to ensure client safety and report behaviors.
