Job summary

Mission Prep Healthcare, part of the AMFM Healthcare family, is seeking a compassionate Community Coordinator to support adolescents at our residential treatment facilities in Fallbrook, CA.

The role includes coordinating client transport, crisis intervention, and collaboration with the clinical team, while upholding Joint Commission standards and ensuring a safe, therapeutic environment. This is a full-time position with competitive pay and benefits.

Qualifications

• Education: High School Diploma or GED; Bachelor's degree preferred.
• Background checks: Must pass federal/state background screenings.
• Valid driver's license with defensive driving and clean record requirements.
• Physical requirements: ability to work at a computer, walk/stand, lift up to 20 lbs.

Responsibilities

• Coordinates and transports clients safely to daily activities and maintains participant safety.
• Utilizes intervention and communication skills with clients and families daily, including crisis moments.
• Adheres to Joint Commission policies and infection prevention/control procedures.
• Collaborates with the program nurse and clinical team on medication management and documentation.
• Maintains residential environments, ensuring cleanliness and safety at all times.
• Document activities in the electronic health record and participate in daily handoffs.
