Job summary

ROWI Teen & Parent Wellness Centers in Rancho Cucamonga, CA seeks a Care Coordinator to oversee coordination of care for all ROWI clients under the Program Director. The role includes orientation, transportation, doctor visits, and scheduling to ensure individualized service delivery.

The position requires facilitating groups, documenting in EMR, coordinating transportation and meals, and maintaining Joint Commission standards with a compassionate, professional approach for clients and families.

Qualifications

• High school diploma required; bachelor’s degree preferred.
• Minimum 2 years driving experience.
• Minimum 1 year working with kids in a therapeutic setting.
• Experience with juniors/high school students preferred.
• Able to communicate with people from diverse backgrounds.
• Capable of coping with stressful situations professionally.
• Strong written communication skills.
• Excellent written and verbal communication.
• Strong interpersonal and organizational skills.
• Ability to work well within an interdisciplinary team.

Responsibilities

• Coordinate care and orientation for ROWI clients.
• Coordinate transportation schedule and drive as needed.
• Document shift reports and share pertinent information with the team.
• Ensure compliance with Joint Commission Standards.
• Attend to group facilitation and activities as directed.
• Maintain confidentiality and professional boundaries with clients and staff.
• Document all transportation and group activity in EMR.
• Submit all documentation in EMR within 24 hours.
• Assist with facility cleanliness as needed.
• Coordinate client lunch daily and support overall program operations.
