Job summary

The LGBTQ+ Center of Las Vegas invites applications for a Case Manager to deliver intensive case management to youth 18-24 experiencing homelessness and help them secure stable housing. You will create individualized service plans, coordinate with landlords, and connect clients with housing resources and other support services.

Provide intensive case management services, including ongoing support, crisis intervention, and life coaching.
Conduct client intake and assessments, ensuring eligibility for the Rapid Rehousing program and completing required documentation (e.g., HMIS).
Develop Individual Success Plans (ISP) with clients, revising and updating plans weekly to reflect progress and goals.
Assist clients in locating housing, completing applications, and understanding lease agreements.
Provide mediation and advocacy with landlords and neighbors to address housing-related issues.
Facilitate referrals for employment, education, mental health support, financial assistance, and other services.
Conduct home visits to monitor client progress, ensure housing stability, and support goal achievement.
Support clients in transitional housing by offering life skills coaching, goal setting, and access to resources.
