Thrive Youth Center is seeking a Case Manager (Full-time) to coordinate assessments and create individualized plans for young adults experiencing homelessness. This on-site role involves hands-on client engagement and collaboration with housing and social services.

You will support shelter and housing transitions, maintain HMIS records, ensure timely documentation, and participate in monthly trainings. The position requires empathy, trauma-informed awareness, and strong teamwork.

Qualifications

• Undergraduate degree or equivalent in social work, human services, sociology or related field.
• Experience in case management with youth, homeless individuals, or diverse communities.
• Commitment to LGBTQ+ affirming care & experience with marginalized youth.
• Training in trauma-informed care, crisis intervention, harm reduction, substance abuse, and mental illness.
• Strong interpersonal skills, emotional intelligence, and compassionate boundaries.
• Ability to collaborate in a diverse, community-based team.

Responsabilités

• Engage clients, conduct intake and assess needs at program entry.
• Develop client-driven case plans to promote self-sufficiency.
• Assist housing search, move-ins, and coordinate with housing dept.
• Maintain HMIS records and complete required documentation.
