The Gay and Lesbian Community Center of Southern Nevada seeks an experienced Case Manager to provide intensive case management for youth (ages 18-24) experiencing homelessness in Las Vegas. You will develop individualized service plans, coordinate with landlords, connect clients to resources, and support Rapid Rehousing and Transitional Housing programs.

Qualifications

• Proficiency in case management, including developing individualized plans and ongoing support.
• Strong knowledge of Housing First, harm reduction, and trauma-informed care.
• Expertise in housing navigation, securing housing and lease understanding.
• Advocacy and mediation skills to resolve landlord or stakeholder conflicts.
• Trauma-informed care principles to support clients with complex needs.
• Harm reduction strategies to promote stability and autonomy.
• Cultural competency to work with diverse populations.
• Excellent communication to build trust with clients and partners.
• Problem-solving and critical thinking to address client challenges.
• Strong organizational skills for multiple tasks and records.
• Proficiency in Microsoft Office, Google Workspace, and HMIS.
• Conflict resolution to de-escalate situations.
• Must pass LiveScan background check and drug screening.

Responsabilités

• Provide intensive case management, including ongoing support, crisis intervention, and life coaching.
• Conduct intake and assessments, ensure eligibility for Rapid Rehousing, complete HMIS docs.
• Develop ISP with clients and update weekly to reflect progress and goals.
• Assist clients in locating housing, completing applications, and understanding leases.
• Mediation and advocacy with landlords to address housing-related issues.
• Refer clients to employment, education, mental health, financial assistance, and other services.
• Conduct home visits to monitor progress and support housing stability.
• Support clients in transitional housing with life skills coaching and goal setting.
• Participate in weekly case conferences with team and partners for progress review.
