Job summary

Catholic Charities of Southern Nevada is seeking a Case Manager-II for the Operation HOME! program. You will provide trauma-informed, client-centered case management, housing and rental assistance, and coordination with community partners.

You will assess needs, develop ISP for each client, and connect them to resources. The role supports move-in and rapid rehousing, while maintaining confidential records and collaborating with providers.

Qualifications

• Associates degree in social service field preferred or equivalent experience.
• Minimum one year of full-time professional casework or homelessness experience preferred.
• Bilingual ability highly desirable.
• Must have valid Nevada driver’s license and auto insurance.

Responsibilities

• Perform comprehensive assessments to identify client needs and develop Individual Service Plans (ISP).
• Coordinate services with project partners and community agencies to ensure planned outcomes.
• Provide housing and rental assistance and support move-in or rapid rehousing options.
• Maintain confidential case notes and documentation across systems.
• Engage with clients in a flexible, client-centered housing approach to meet needs.
• Follow through with referrals to ensure successful service delivery.
