Job summary

TOGETHER! seeks a Temporary Host Homes Case Manager to coordinate wraparound social services for youth and Host Families within the Host Homes Program. You will support housing stability, education, employment, financial literacy, and health, while collaborating with schools and community partners.

The role includes managing a caseload of 8–10 youth, delivering trauma-informed care, and coordinating service plans across program sites in Thurston County. Travel and training are expected.

Qualifications

• Bachelor's degree in Community Health, Social Work, Psychology, Social Sciences, Education, or closely related field.
• Minimum of one year of employment experience in case management, social services, health, or education.

Responsibilities

• Provide specialized services to youth and young adults in education, employment and housing.
• Manage a caseload of approximately 8–10 youth throughout the program.
• Provide case management to Hosts throughout the program lifecycle.
• Build relationships with participants, focusing on mentorship and trauma-informed care.
