Job summary

Compass Career Solutions in Tacoma, WA is seeking a Housing Coordinator to conduct assessments, develop action plans, and provide life skills development for clients facing homelessness. The role requires in-office work, strong documentation, and collaboration with the team to support housing stability.

You will connect clients to medical and social services, assist with program applications, and travel within the area to support clients and conduct wellness checks.

Qualifications

• Bachelor's Degree in social services or related field with 1+ year housing coordination experience
• Or High School Diploma or GED with 2+ years housing coordination experience
• Current resume includes bulleted details of direct housing experience
• Reliable, flexible schedule; valid driver's license and vehicle
• Ability to lift up to 50 lbs; CPR/First Aid

Responsibilities

• Conduct assessments and interviews to determine eligibility
• Develop and update action plans and case notes
• Provide ongoing life skills development to improve independence and quality of life
• Document care and collaborate with internal/external team members
• Provide interventions when clients risk eviction
• Connect clients to medical and mental health services
• Establish a network of community services and referrals
• Guide clients through SSI/SSDI/Medicaid/Medicare/SNAP applications
• Occasionally visit clients at home
• Coordinate with property management for wellness checks
• Track and report outcomes and provide data updates
• Attend internal and external meetings
• Travel to assist clients and provide transportation
• Represent Compass at meetings and in the community
• Utilize assigned technology (laptop, cell phone, software)
