Job summary

Hudson River Housing, Inc. is seeking a Housing Care Manager to provide comprehensive services within our youth-focused programs in New York. You will conduct intake, develop goal-oriented service plans, and advocate for participants to access needed services and entitlements.

Responsibilities include counseling, referrals, coordination with partners, and documenting progress. A strong focus on ethics, cultural sensitivity, and independent teamwork will guide daily activities and program

Qualifications

• BA/BS in human service field or equivalent combination of education, training and experience.
• Two years related work experience with youth population.
• Competent reading and writing skills along with strong communication skills.
• Excellent organizational skills; able to prioritize demands; respectful of cultural differences; able to work independently and in a team.

Responsibilities

• Provide general information about the program and conduct intake interviews to assess needs.
• Develop goal-oriented service plans with participants and families as appropriate.
• Offer guidance and support to help participants meet basic needs and achieve goals.
• Facilitate referrals and advocate to secure services and entitlements.
• Collaborate with staff and outside providers to coordinate participant needs.
• Provide individual, group, and family counseling as aligned with the plan.
• Assist with planning workshops and activities to build living skills and support networks.
• Monitor progress and adjust goals as needed; document case notes and reports.
