Job summary

Hudson River Housing is seeking a dedicated Housing Care Manager in New York. The role involves delivering services, guiding participants, and coordinating care within assigned programs, with emphasis on crisis intervention, intake, and goal-driven service plans.

You will collaborate with families and providers, offer counseling, arrange workshops, and monitor progress toward goals while ensuring ethical standards and confidentiality.

Qualifications

• BA/BS in human service field or equivalent combination of education, training and experience.
• Two years related work experience with youth population. Reading and writing skills and strong communication needed.
• Excellent organizational skills with ability to prioritize demands.
• Sensitivity toward diverse populations and ability to relate to individuals from diverse backgrounds.
• Responsible, non-judgmental; able to work independently and in a team.
• Commitment to ethics and maintaining boundaries with participants.
• Dependable and flexible.

Responsibilities

• Provide information about the program, crisis intervention, and screen applicants.
• Orient participants to program and conduct intake interviews.
• Develop goal-oriented service plans with participants and families.
• Offer guidance and support to meet basic needs and achieve goals.
• Facilitate referrals and advocate for participants to obtain services and entitlements.
• Collaborate with staff and external providers to coordinate care.
• Provide individual, group and family counseling per case plan.
• Arrange workshops, support groups, and activities to build living skills and support systems.
• Monitor adherence to program rules and intervene in crises.
• Track progress toward goals and adjust plans as needed.
• Participate in interviews, reviews, trainings, and agency events.
• Maintain confidential files and records; support discharge planning and follow-up.
• Provide office and program management support; contribute to program evaluation.
• Engage in ongoing training for professional growth.
