Job summary

S:US in New York is seeking a Community Coordinator to help clients access housing, health care, and support services. You will coordinate telehealth visits, assist with appointments, and connect individuals to resources through the ACT Teams.

The role emphasizes compassionate, person-centered care, teamwork with providers, and accurate documentation. You will schedule, follow up, and support with transportation and referrals to ensure smooth transitions to housing and ongoing wellness.

Qualifications

• HS Diploma/GED and/or Peer Certification required; Bachelor's degree in Mental Health, Social Work or related field preferred.
• Experience in the Mental Health service field preferred.

Responsibilities

• Facilitates telehealth video calls for team providers monthly/quarterly.
• Assist with tele-visits to ensure clients are trained to do telehealth.
• Create a wellness action plan tailored to each consumer’s needs with providers and community organizations.
• Provide clients with ongoing follow-up regarding graduation and housing goals.
• Support the team with telehealth and/or field visits (10-12 per month max).
• Assist with 2-person visits for high risk clients.
• Escort clients to AOT appointments.
• Seek out community medical and mental health resources and provide referrals.
• Assist clients with medication review and directives to reduce hospital utilization.
• Coordinate transportation for medical appointments and provide reminders.
• Support around recreational activities.
