Job summary

Haitian Centers Council (HCC) in New York, NY seeks a Health Navigator / Outreach Worker to engage YMSM, deliver rapid HIV testing, and connect participants to prevention and care services. The role emphasizes cultural affirming support, addressing barriers, and providing navigation for PrEP, PEP, STI testing, and related services.

The position includes group interventions, drop-in activities, and data documentation while maintaining HIPAA compliant practices in a welcoming, safe environment.

Responsibilities

• Outreach & Community Engagement: Conduct outreach at least three times weekly targeting YMSM in person and online.
• Testing & Screening Services: Provide rapid HIV and Hepatitis C testing; conduct pre/post-test counseling.
• Navigation, Referrals & Linkage to Care: Provide health navigation for PrEP/PEP, HIV/HCV care, and support services.
• Group-Level Interventions & Education: Co-facilitate evidence-based interventions and recruit participants.
• Drop-In Activities & Wellness Center Support: Facilitate engaging drop-in activities; maintain stocked materials and safe environment.
• Data Entry, Documentation & Quality Assurance: Enter data weekly; ensure confidentiality and accuracy.
• Training & Professional Development: Attend trainings and participate in supervision and meetings.
